NDIS Support Coordination -

Support Coordination at Capacity Building Co will assist with planning and managing NDIS Plan. Our motivated Support Coordinators provide support to implement your NDIS plan by:

• Helping you understand your plan

• Finding services and supports in your local area

• Assisting you to choose and connect with service providers

• Working collaboratively to explore local community and mainstream services, and help coordinate these as required

• Provide advice on how to manage your condition or situation best by connecting you with the right healthcare providers and support services. They will also negotiate with the service providers about the range of services given plus their cost.

• Talking with you and your family or loved ones about alternative options as the year progresses and facilitate the development of future goals for your next plan

• Act as your NDIS contact person, available to discuss any questions or provide assistance to resolve any problems if they arise

• Provide advocacy for you in NDIS plan reviews and in the community. 

At Capacity Building Co we facilitate access to either Support Coordination, Specialist Support Coordination or Psychosocial Recovery Coaching.' We provide services to all areas of Greater Sydney NSW .

Psychosocial Recovery Coaching -
